Print and fill out the COLLEGE PETITION FORM
Print and have your professor (s) fill out the Late Withdrawal/Drop Request Form
Write a personal statement addressing the reason for your petition
Make an appointment to meet with Dr. Johanson

Pre-Health Professional Students
After you get the SCORE for your Admission Exam (MCAT, DAT, etc) make sure you fill out the REQUEST FOR PRE-HEALTH PROFESSIONS COMMITTEE MEETING form in SE 234.
